Schedule Appointment
& Create Account
Important Notes:
- If you have problems with the instant online
scheduling system, you may request an appointment by completing a
manual appointment request form. (Note: Use this form only if
you experience challenges with the above automated system.)
- If you previously completed Step #1 (the Intake Form) but
did not schedule an appointment, you do not need to complete another
intake form. Just proceed to 'Schedule Appointment & Create Account'.
